Cancellation and returns policy
Policies:
1. One person must be designated by the family who will be responsible for communicating with the home health care department.
2. Patient data must be updated in case of change in contact numbers or home address.
3. The patient/family must treat Home Med staff in a decent manner.
4. A caregiver/family member must be present during the therapy session who can ask questions regarding the patient’s care/condition but must not interfere in the therapy session.
5. The patient must be ready and clean for the session before the home health care team arrives.
6. Please eat your meal and take your prescribed medications before the session as directed by the doctor in charge of the case.
7. The family must prepare a place for the home health care team where they can provide their services without interruption such as the presence of a television, family visitors, or other family members during the session, in order to protect the patient’s privacy.
8. The patient must remain in one home during the treatment period: as the continuity of treatment - in the event that the patient changes his location - depends on the possibility of having appointments in the new area.
9. Your length of stay depends on the medical recommendations of the treating medical team and is subject to change according to your medical and rehabilitation needs during the program.
10. The team will give you a (service consent form) that you sign at each visit to confirm your consent to the service provided to you by the service providers.
Cancellation Policy:
1. In case of wishing to cancel in advance or change the date of a visit during the contract period, we appreciate you notifying us at least 24 hours before the scheduled visit time, excluding weekends. In case we are notified less than 24 hours before the visit time, the full cost of the visit will be charged to the customer’s account.
2. In case of wishing to cancel the entire contract - before the start of the treatment plan implementation - the cancellation request must be submitted one week before the start of the treatment plan, and if the cancellation request is submitted less than a week before the start of the contract, a cancellation fee of 5% of the total contract value will be calculated and the remaining balance will be returned to the customer - if any -.
3. In the event of a desire to cancel the contract and offers during the contract period or the provision of services and the full value of the contract or offer has been paid by the client - the treatment plan has been implemented - the value of the services provided to the patient will be recalculated from the beginning based on the original price of the services before the discount and the remaining amount will be calculated as a credit for the client.
4. In the event that the client wishes to stop the treatment plan during the implementation period and change the service to another service, the cancellation conditions mentioned above will be applied, and then the remaining balance of the patient - if any - will be transferred to the required service.
5. In case the appointment is cancelled after the team arrives at your home, the full cost of the visit will be charged to the customer.
6. The visit will be cancelled if the door of the house is not opened and the team waits for more than 15 minutes.
7. All requests for appointment changes or cancellations are subject to the evaluation of the treating physician to ensure the effectiveness of the treatment plan and to achieve the maximum benefit for the client.
8. Any service provided to the customer by a third party (laboratory tests - x-rays - others) is subject to the third party’s cancellation policy and the terms and conditions mentioned above do not apply to it.
9. Services that require pre-arrangement, such as intravenous vitamins, will not be cancelled.
Conditions under which your home health care program will be stopped:
1. If your goals are met.
2. If you are hospitalized for more than 24 hours.
3. If the cancellation occurs after the care team arrives 3 times in a row without notifying the Home Health Care Department.
4. If you are traveling/or have been stopped for more than 5 days, which requires a new referral if follow-up is required.
5. Any unethical behavior by the patient/family towards home health care staff.
